The HOPE team is characterised by a strong commitment to our consumers, their families/carers, and our service provision partners, ensuring the highest standards of care.
We encourage individuals with lived experience of utilising mental health services due to suicidal ideation or behaviours, and/or those who are carers of individuals with similar experiences, to apply. This is a full-time role. The clinic is based in Melton, and the HOPE team will work closely with the broader community mental health team in the region.
The HOPE initiative was developed to provide support to individuals discharged from the Emergency Department, medical wards, or inpatient units after a suicide attempt, or those displaying serious suicidal ideation, intent, or planning. This service covers the Melton, parts of Hume (Sunbury), and Brimbank catchment areas. HOPE will support clients for a period of 12 weeks, offering assertive and coordinated care post-discharge. Support will include both outreach and clinic-based therapeutic interventions aimed at reducing the risk of re-attempts and deaths by suicide.
