Wesley Mission Queensland is seeking a Care Partner to support older Australians to live independently at home. You will develop, implement, and review personalised care plans and coordinate services, while building trusted relationships with customers, families, carers, and health professionals.
A Certificate III in Individual Support (Ageing) or Certificate IV in Aged Care is required, with a minimum 12 months' experience in aged care or related services.
