You’ll work at the heart of community‑based care, partnering closely with customers, families, and multidisciplinary teams to deliver responsive, high‑quality services that truly make a difference.
At WMQ, we are a values‑driven, not‑for‑profit organisation committed to care with integrity, compassion, and purpose.
Your Role as a CHSP Care Partner:
- Based in Chermside, you will effectively and efficiently manage a defined caseload of Commonwealth Home Support Program (CHSP) customers, ensuring the delivery of responsive, coordinated, and high‑quality community aged care services.
- You will work collaboratively with customers, carers, families, advocates, and internal and external stakeholders to design and review detailed care plans aligned to customer goals, maximising independence and community participation.
- This role plays a key part in ensuring services are delivered in line with WMQ policies, the Aged Care Quality Standards, and relevant funding requirements, while also supporting service sustainability and growth.
In this role, you will:
- Act as the primary point of contact for customers and their supporters, delivering person‑centred customer service.
- Develop, review, and coordinate individualised care plans focused on wellness and independence.
- Maintain regular and meaningful communication with customers, families, and stakeholders to ensure services remain aligned with needs and goals.
- Identify changes in customer circumstances and respond quickly, adjusting services and facilitating referrals where required.
- Work closely with clinical and allied health teams to ensure coordinated and holistic care.
- Coordinate services, equipment requests, and procurement to ensure continuity of care.
- Monitor, record, and respond to incidents, risks, and customer feedback in a timely and professional manner.
- Maintain accurate documentation, data entry, and reporting in line with legislative, funding, and WMQ requirements.
- Participate in care team meetings, case conferences, and continuous quality improvement activities.
- Promote WMQ services and build strong networks to support business growth and positive customer outcomes.
What We’re Looking For:
You are a compassionate, organised, and customer‑focused professional who thrives in a fast‑paced, community‑based environment. You communicate clearly, build trust easily, and are passionate about supporting older people to live well at home.
Essential:
- 3-5 years’ experience in aged care, community services, or care/case management.
- Minimum Certificate III in Aged Care, Community Services, or a related field.
- Experience in customer engagement, service coordination, and working within regulated funding models (e.g. CHSP, aged care).
- Strong understanding of Consumer Directed Care, wellness, and reablement principles.
- Highly developed organisational and time‑management skills.
- Solid computer literacy, including Microsoft Office and client management systems.
- Current Driver’s Licence and willingness to travel as part of the role.
Desirable:
- Experience facilitating case conferences or multidisciplinary collaboration.
- Experience working with diverse or culturally and linguistically diverse communities.
- Strong data and reporting capability to monitor customer outcomes and service delivery.
At WMQ, our work is guided by integrity, respect, empowerment, hope, justice, compassion, and innovation.
