About the role

You will visit clients at home, assess their nursing needs, develop and review individualised care plans, and deliver care within your scope of practice.

This role combines direct client care with clinical documentation, assessment reports and follow‑up.

You will work with our administration and operations teams to coordinate your workload and communicate clearly with clients, families and other healthcare professionals.

  • Plan your workload, meet agreed deadlines and communicate any barriers to timely completion.
  • Travel between clients within your allocated service area.

Experience with PEG Tube changes, diabetes management, stoma care and pressure injury prevention and management will also be valued.

About you

You are comfortable working independently in clients’ homes, while recognising when to seek advice or involve other healthcare professionals.

You combine sound clinical judgement with clear communication and reliable organisation.
