Uniting is seeking an experienced Neighbourhood Care Partner to join our Home and Community Care team.
You will be responsible for case management, care planning and coordinating person-centred services that support clients to remain living in their own homes for longer.
You will work with clients and their networks to tailor services to individual needs and funding, travelling across Dubbo and surrounding regions as required.
