You will coordinate and oversee individualised services that support clients to live safely and independently in their own homes, using your nursing expertise to assess needs and plan care.
You'll mentor carers, develop care plans, manage budgets and collaborate with GPs and allied health providers to ensure safe, client‐centred care.
