Here's what you can expect:

  • Not-for-profit salary packaging ($15,900 per annum)
  • Leave loading to make your time off even more rewarding
  • Flexible and hybrid working options
  • Clear, supported career pathways and training to help you grow

  • Employee Assistance Program - confidential support when you need it
  • QLeave - portable long service leave that recognises long-term care work

This is a permanent full-time role, with Monday to Friday hours.

What you'll be doing

As a Care Partner, you'll help people stay in the place they're comfortable, connected to their community, and living life on their terms. You'll be the person customers and families turn to for guidance, support and practical solutions. From developing personalised support plans and coordinating services, to navigating changing needs and responding when challenges arise, you'll play a key role in helping people continue living the life they love. No two days look exactly the same. One moment you might be helping a customer access a new service, the next you'll be supporting a family through an important decision or working alongside healthcare professionals to make sure the right care is in place. Behind the scenes, you'll keep documentation accurate, manage budgets and funding requirements, and contribute to continuous improvement. Most importantly, you'll build genuine relationships with the people you support, because great care starts with trust.

This is a full-time permanent position (Monday to Friday, 8:30am - 4:36pm) supporting customers across the Sunshine Coast region. You'll enjoy a hybrid work arrangement, combining time in our Maroochydore office with working from home.

You'll thrive in this role if you:

  • Like building relationships, solving problems, and helping people navigate life's moving parts.
  • You know how to balance compassion with action. You're comfortable having meaningful conversations, can think on your feet when plans change, and genuinely care about helping people achieve what matters most to them.

You'll also bring:

  • A qualification in social, human or community services, such as a Diploma of Community Services or Certificate IV in Aged Care, or equivalent experience supporting people in community-based settings
  • Experience developing support plans, coordinating services and managing care arrangements
  • Strong communication skills and the confidence to navigate sensitive conversations with warmth and professionalism
  • Excellent organisational skills and the ability to manage competing priorities
  • Experience working collaboratively with customers, families and service providers to achieve positive outcomes
  • Knowledge of local support services and, ideally, Home Care Packages

Belonging Starts Here

We believe real care starts with real belonging. That means creating a workplace where everyone - no matter their background, culture, or story - feels seen, respected, and safe to be themselves. We welcome applications from First Nations people, the LGBTIQA+ community, people with disabilities, and those from culturally diverse backgrounds. Your story matters here. We proudly work across the lands of the Gubbi Gubbi (Gub-bee Gub-bee), Turrbal, Yuggera, Darumbal, Butchulla (Batjala) & Taribelang Bunda peoples, and extend our respect to all Aboriginal and Torres Strait Islander peoples and communities. We are committed to creating a culturally safe, inclusive and welcoming workplace for all.
