Stable Home Care is seeking an experienced Care Partner to coordinate care for clients under the Support at Home program on the Gold Coast. You will liaise with clients, families, and health professionals to ensure services align with goals and budgets, delivering person-centred care.
You will lead assessments, develop care plans, monitor budgets, and supervise staff, with a focus on safety, independence, and quality. AHPRA registration or allied health background is preferred.
