St Vincent's Hospital Melbourne is seeking an experienced Social Worker to join the ALERT team as an Emergency Department Care Coordinator. You will coordinate care for patients with complex health and social needs, perform comprehensive psychosocial assessments and plan safe discharges from the ED.
You will also extend support into the community, providing case management and linkage to hospital and community services. A health focus on homeless and HIP/HfH programs underpins the role.
