About the role: The Pathways to Community Living Initiative (PCLI) was established to support the transition of long-stay mental health patients into appropriate community-based living and services. PCLI’s aim is to transition long-stay patients - or those at risk of long stays - who experience very complex mental health issues back into the community and create a practice change in inpatient and community services across NSW to decrease the number and length of long stay admissions. This position will work within a multidisciplinary team to provide psychiatric consultation, treatment, care planning and clinical oversight of patients referred to the PCLI team using person-centred, recovery-oriented, family/carer-inclusive and trauma-informed principles of care. Collaborative engagement with other clinicians will be crucial in influencing practice change across the mental health service. This position will act also as a senior clinician providing leadership within the multidisciplinary team.
