About the role

This is community nursing the way it should be: relationship-based, flexible, and genuinely rewarding. • Coordinating with families, GPs, and allied health teams
  • Supporting clients to live well at home
  • Building trusted long-term relationships
  • Documentation and care coordination

You’ll work independently throughout the community while being backed by an experienced and supportive leadership team who genuinely understand nursing.

We are seeking someone who:

  • Loves relationship-based care
  • Is calm, organised, and clinically confident
  • Enjoys working autonomously
  • Communicates with warmth and professionalism
  • Wants to make a real difference in the community
  • Is passionate about supporting older people to live well at home

  • National Police Clearance
  • Strong clinical assessment and documentation skills
  • Confidence using clinical software and IT systems
  • Annual influenza vaccination

Highly Regarded:

  • Community nursing experience
  • Aged care or palliative care background
  • Wound management and chronic disease experience
  • Experience working independently in complex clinical settings (We’re seeking nurses with 2+ years experience.)
