Southern Cross Care WA is seeking an experienced Therapy Assistant to join our Community Transition Care Program in the South West Metro as part of our Home Care team. You will work with older people, deliver person-centred therapy, manage a caseload and support care teams to help clients reach mobility and independence goals.
You will assist allied health professionals, guide exercises, maintain records, and follow safety guidelines.
