Silverchain is seeking experienced Home Care Workers to join our Transitional Aged Care Program (TACP) in the Manly area, NSW. You will provide home care support to older clients transitioning from hospital to independent living and help them achieve recovery goals.
The role offers a part-time roster (Mon–Fri, 7am–5pm, 2–3 days/week) with award rate pay, superannuation, and training opportunities, including CPR/First Aid. Prior post-hospital care experience is highly valued.
