Scope is seeking a Service Transition Coordinator to guide new clients into Scope's Supported Independent Living (SIL) services. You will coordinate onboarding, gather client goals, and liaise with families and internal teams to ensure a smooth start to services.
You will work with diverse stakeholders in a fast-paced environment, maintaining accurate records and complying with funding and regulatory standards while promoting inclusion and positive client outcomes.
