Make a Real Difference as a Senior Social Worker! The Virtual Rehabilitation Ward at Flinders Medical Centre provides care to people with complex medical, functional, and psychosocial needs in their own home - in place of an extended or avoidable hospital admission.
About the role:
- Reporting to the Manager, this role is responsible for providing specialist social work assessment, intervention and care planning to people with complex medical, functional and psychosocial needs - supporting them to remain in their own home in lieu of an extended or avoidable hospital admission.
- Commitment to delivering high quality and safe care consistent with the SALHN Integrated Governance Framework and Consumer Engagement Framework and Plan.
- Independently assessing clients' abilities, problems and needs using a variety of formal and informal assessments as needed.
- Communicating with clients' family, carers and other service providers.
- Participating in interdisciplinary clinical and planning activities to enhance the overall service provided to clients.
- Initiating, developing and contributing to educational activities within the team.
About you:
- A degree or equivalent in Social Work, which gives eligibility for full membership of the Australian Association of Social Workers.
- Demonstrated competence in the application of a wide range of assessment and therapeutic interventions to clients with complex needs.
- Ability to critically analyse or evaluate issues related to clinical care in social work in a manner which can be applied to quality activities, service evaluation or research.
- Experience providing social work services to people in a hospital setting.
- Proven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards.
