As a Mental Health Practitioner, you will provide flexible, outreach-based care for young people aged 12-25 who are experiencing significant and complex challenges, including severe mental illness or the risk of developing it, and disconnection from mainstream services and supports. Working closely with a Functional Recovery Manager in a two-worker model, you will deliver integrated clinical and psychosocial support that promotes recovery, engagement, social participation and long-term well-being. The role combines advanced clinical practice with mentoring and service innovation, grounded in a relational, person-centred, trauma- and poverty-informed model of care. Services are often delivered in homes, schools, parks, cafés, courtrooms, and other community settings. You will participate actively in multidisciplinary team case reviews, coordinate care effectively, and occasionally facilitate therapeutic group sessions.
Key responsibilities:
- Prioritise relationship over intervention to young people and families experiencing distress, adversity, or risk of disconnection.
- Lead with curiosity and care in outreach settings, meeting young people where they are.
- Co-create flexible, youth-led treatment plans that evolve over time, guided by trust, engagement, and the young person's own goals and pace.
- Subtly apply psychological theory and methods to assess, understand, and support young people with complex needs.
- Hold purposeful boundaries while offering unconditional positive regard, modelling relational authenticity and cultural humility.
- Contribute to program planning, reflective practice, and inter-agency collaboration.
- Commit to a five-day working week (38hr) with the flexibility required to meet young people in community, including some work outside standard hours.
- Participate in multidisciplinary team case reviews and coordinate care effectively.
About you:
- Relational and clinically skilled: You have experience and a strong commitment to combining clinical expertise with deep relationship-building and responsive, trauma-informed care.
- Grounded in values: You prioritise connection, presence and integrity in complex work, and model these qualities for others.
- Reflective and resilient: You effectively manage emotional demands, sustain your well-being and embed self-care and reflective practice into your routine.
- Inclusive and responsive: You bring cultural humility, a strong commitment to equity and the ability to engage inclusively across diverse communities.
- Collaborative and proactive: You work well with colleagues, partners and systems, contributing to learning, innovation and continuous improvement.
- Comfortable with complexity: You can hold multiple truths, consider different perspectives and sit with tension, uncertainty and discomfort.
Skills and experience:
- Relevant four-year degree with two years' mental health-specific experience, or a three-year degree with three years' relevant mental health experience (psychology, mental-health-accredited social work, occupational therapy, mental health nursing, or equivalent).
- Demonstrated knowledge and experience in a range of therapies, including trauma-informed, stepped-care, and recovery-oriented practice.
- Experience in working with young people experiencing barriers to accessing support and a commitment to a relational model of care.
- Ability and commitment to build authentic relationships with young people who have historically struggled to engage with traditional mental health services.
- Proven experience working collaboratively within multidisciplinary teams.
- A clear commitment to child-safe practice, including listening to young people, respecting their rights and boundaries, recognising and responding to concerns, and helping create an environment where safety is everyone's responsibility.
Practical requirements:
- Appointment is subject to RAYS' child-safe recruitment and screening requirements, including relevant reference and background checks.
- You will also need a current Queensland driver licence.
- A current Blue Card or the ability to obtain one.
- A current First Aid and CPR certificate or willingness to obtain these.
