Support at Home Care Coordinator (Care Partner) at Pearl Home Care Gold Coast | Hope Island
Permanent full-time | Hybrid, with work-from-home days
Are you an experienced Home Care Package or Support at Home coordinator who wants a caseload you can manage? At Pearl Home Care Gold Coast, our Care Partners are the Support at Home coordinators for their own group of clients. You’ll set up each client’s services, build and review their care plans, manage their quarterly budgets, and be the main contact for them and their families. This is an office-based coordination and case management role. It is not a support worker position.
About the role:
- Onboarding new clients: reviewing their My Aged Care referral and classification, holding the initial assessment, and setting up the service agreement and care plan.
- Care planning: building care plans around each client’s goals, and reviewing them regularly and whenever needs change.
- Budget management: managing quarterly budgets, explaining client contributions and monthly statements, and making sure funds are used well and not left unspent.
- Coordinating services: working with our scheduling team, support workers, allied health and third-party suppliers.
- Short-term pathways: coordinating the Restorative Care Pathway, the End-of-Life Pathway and the AT-HM scheme (assistive technology and home modifications).
- Reassessments: spotting when a client’s needs have outgrown their classification and arranging a reassessment.
- Clinical concerns: recognising changes in health or risk, and escalating to our Clinical Care Manager and RN.
- Client contact: keeping in regular contact with clients and families, and being the person they call.
- Records and compliance: keeping accurate care management notes and meeting the Aged Care Quality Standards and Support at Home program rules.
About you:
- You have at least 2 years’ experience coordinating Home Care Packages or Support at Home services.
- You know how classifications, budgets and care management work, and you can explain them clearly to a client or family who finds them confusing.
- Strong care planning, organisation and time management across a full caseload.
- A working knowledge of the Support at Home program manual, the Aged Care Quality Standards and client rights under the new Aged Care Act.
- Good clinical judgement in recognising risk and complex needs, and knowing when to elevate.
- Clear, warm communication with clients, families, support workers and health professionals.
- Confidence with client budgets, care management software and Microsoft 365.
- Experience with dementia, palliative care, hospital discharge or restorative care is highly regarded.
Qualifications and requirements:
- Minimum: 2 years’ Home Care Package or Support at Home coordination or case management experience.
- Preferred: a nursing (RN/EN) or allied health qualification.
- Overseas-qualified nurses and allied health professionals are welcome to apply. AHPRA registration is not required for this coordination role.
- Current First Aid and CPR.
- Current driver’s licence and a comprehensively insured vehicle (mileage reimbursed for client visits).
- Current National Police Check, or willingness to get one.
Work-from-home days once you’re settled in. Free parking at our Hope Island office. A caseload you can manage. We add Care Partners as we grow, so you can know your clients properly. Clinical backup from our in-house Clinical Care Manager, RN and OT. A small, local team. The owners are in the office, so ideas are heard and decisions are made quickly. Room to grow, with training and development and a real say in how we improve our service.
