Based across our Supported Independent Living Facilities, you will drive quality clinical care plan development and review, in collaboration with a multi-disciplinary team.
You will assess new clients, formulate goals within the Peace of Mind Framework, and ensure compliance with clinical and legislative requirements, including NDIS High Intensity Daily Personal Activities.
