Location: Darwin
Primary Objective: Promote and support the relationship between NT Health, NT Primary Health Network and Primary Care providers in the NT. The role is primarily located within Royal Darwin and Palmerston Hospital and is responsible for pro-actively engaging with both RDPH clinicians, GPs and other primary care providers across the Top End to strategically improve communication, collaboration and integration between primary care and tertiary care with the focus on person-centred care. This is a part time vacancy available for 0.5 (19 hrs) per week.Selection will be made at the SSC or SSSC level depending on the experience/qualifications of the successful applicant.
