This position qualifies for an annual Recruitment Incentive Package up to the value of $10,000 (conditions apply and may be pro-rata’d to your contracted or ordinary working hours, these payments are paid on a fortnightly basis) under the NSW Health Rural Health Workforce Incentive Scheme. This scheme recognises the role key health workers play in enabling the delivery of rural and remote health services, and aims to provide incentives over and above award entitlements, to attract and retain staff in these areas.

What you'll be doing:
- The Aged Care Assessor / Specialist Registered Nurse will provide comprehensive assessment services and care planning to determine the overall needs of frail, older people in community, residential and hospital settings within the FWLHD.

Key Requirements:
- Current registration to practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
- Demonstrated post graduate experience working with older people in an acute, residential or community setting.
- Demonstrated ability to work independently and in an effective manner encompassing appropriate initiative, organisational, problem solving and time management skills.
- Proven written and verbal communication skills including demonstrated computer literacy.
- Demonstrated ability to work autonomously as well as function as a member of a multidisciplinary team.
- Understanding or experience in the health and functional support needs of older people and their carers in the community setting.
- Current Drivers Licence (minimum class C) valid within Australia and demonstrated competent driving ability/experience.
- Current Working with Children Check, Aged Care Check and NDIS check.
