Key Duties and Responsibilities Client Assessment & Care Planning
- Conduct comprehensive assessments of residents' physical, social, and emotional needs, and collaboratively develop individualized care and support programs.
- Interview residents and their families to identify the nature and extent of difficulties, adjusting care plans accordingly.
Program Implementation & Monitoring
- Implement educational, training, and lifestyle programs that promote engagement and wellbeing, including social activities and health education.
- Monitor residents' progress routinely, documenting changes and reporting outcomes to the clinical team and families.
Community Liaison & Referral
- Identify community resources and services such as health, welfare, housing, and allied health and refer residents and families to additional support.
- Liaise with community groups, government bodies, and private providers to raise awareness of available services and advocate for residents' needs.
Reporting & Funding
- Prepare detailed submissions for funding and resources, ensuring compliance with Aged Care standards.
- Report to government agencies and other bodies on resident outcomes, service gaps, and program effectiveness.
Requirements
- Diploma or higher in Community Services, Aged Care, or related field.
- Minimum of 1 year experience in aged care assessment, care planning, and case management.
- Strong communication skills, with ability to engage families, agencies, and multidisciplinary teams.
- Knowledge of My Aged Care, funding frameworks, and reporting requirements.
- Current police check and driver's license.
