Are you a talented Dietitian looking to make a significant impact within regional communities?

We are seeking a passionate and motivated Dietitian to join our multidisciplinary team within the Transitional Aged Care Program (TACP), supporting our aged care patients/clients as they transition from hospital back to their homes across the Wagga Wagga region.

In this rewarding community-based role, you will provide comprehensive nutrition assessments, develop individualised nutrition care plans, deliver tailored interventions and review outcomes to support clients’ recovery, wellbeing and independence. You will manage a diverse caseload, supporting up to 25 TACP packages (subject to your hours), and conduct home visits to work collaboratively with clients, their carers and other service providers to address individual nutritional needs and goals.

Working closely with an experienced multidisciplinary team, including Occupational Therapists, Physiotherapists, Registered Nurses, Allied Health Assistants, Social Workers and the TACP Coordinator, you will contribute to goal-focused care planning, case conferences and multidisciplinary reviews to ensure coordinated, person-centred care.

This is an excellent opportunity to apply your dietetic skills in a supportive team environment while helping older people maintain their health, independence and quality of life in their own homes.
