Social Worker, Grade 4 - Agile Psychological Medicine (APM)

The Agile Psychological Medicine (aPM) Clinics provide personalised, flexible, and evidence-based therapeutic care to consumers accessing public mental health services. Working as part of a multidisciplinary team of Psychologists, Social Workers, and Psychiatrists, you will play a key role in supporting consumers experiencing acute mental health challenges and helping them navigate their recovery journey.
In this role, you will:
- Conduct comprehensive psychosocial assessments for consumers referred through psychiatric triage, emergency departments, and inpatient services
- Work collaboratively with consumers to identify their needs and develop recovery-focused treatment plans
- Provide therapeutic social work interventions and system-based support to improve outcomes and promote recovery
- Utilise routine outcome measures to inform clinical decision-making and enhance care delivery
- Contribute to a supportive and innovative multidisciplinary team committed to evidence-based practice and continuous improvement
About You
You are a motivated and compassionate Social Worker with experience in mental health and a commitment to delivering high-quality, person-centred care.
You will bring:
- Experience working within mental health settings
- Strong assessment, engagement, and therapeutic intervention skills
- An interest in systems-based and/or family-inclusive practice
- The ability to work independently while contributing effectively within a multidisciplinary team
- A flexible and adaptable approach to managing a dynamic clinical workload
- A commitment to evidence-based practice and using outcomes to inform care
