Cura Aged and Disability Care Services is a leading not-for-profit organisation dedicated to delivering culturally responsive home and community-based support to older people, people with disabilities, and their carers. Our mission is to empower individuals to live independently, maintain their wellbeing, and remain connected to their communities.

We are seeking an experienced, compassionate, and values-driven Care Partner to join our team. In this role, you will be responsible for delivering high-quality case management, support planning, and care coordination services across a range of home care programs. Working closely with participants, families, and service providers, you will ensure supports are tailored to individual needs, goals, and cultural preferences.

As a Care Partner, you will be responsible for onboarding, coordinating, monitoring, and reviewing person‑centred home care services for older Australians receiving services under the Support at Home (SAH) and Commonwealth Home Support Programs (CHSP). Working within the Strengthened Aged Care Quality Standards and Aged Care Act (2024), you will support participants to maintain their independence, wellbeing, choice, and control while living safely in their own homes.

What will you do:

  • Build strong participant relationships through assessments, reviews, care planning, and regular contact to ensure services align with individual goals, needs, preferences, and rights.
  • Coordinate and monitor services ensuring supports remain responsive, effective, culturally appropriate, and delivered within the allocated quarterly funding and program requirements.
  • Promote participant choice and rights, supporting informed decision‑making, access to advocates, community services, and culturally safe, inclusive care.
  • Manage quality and compliance by maintaining accurate documentation, care plans, case notes, participant reviews, workplace safety assessments, and quality records in AlayaCare.
  • Identify and respond to risks, escalating clinical concerns appropriately and participating in incident management, complaints handling, and safeguarding activities.
  • Collaborate with participants, families, Home Care Workers, health professionals, and service providers to deliver coordinated, high-quality care and support.
  • Contribute to continuous improvement, audits, professional development, service growth initiatives, and the delivery of high-quality aged care services aligned with the Strengthened Aged Care Quality Standards.
  • Participate in the On-Call and Duty Care Partner rosters in line with business requirements.

What you will need:

  • Minimum Certificate IV level qualification in Aged Care, Disability, Community Services, Nursing, Allied Health or a related field.
  • Minimum 2 years' experience in case management, care coordination or a similar role.
  • Demonstrated experience in participant assessment, case management, care planning and budget management.
  • Sound understanding of Consumer Directed Care and the aged care reforms, including the Aged Care Act (2024) and Strengthened Aged Care Quality Standards.
  • Knowledge of aged care, disability and community services funding models, including experience working with Support at Home, Commonwealth Home Support Program, NDIS and other government-funded programs.
  • Knowledge of aged care services and referral pathways.
  • Excellent communication, customer service and relationship building skills.
  • Current National Police Check, Blue Card, First Aid, and CPR certificates (or willingness to obtain).
  • Current Driver's Licence and reliable vehicle.
  • Strong computer literacy and experience using client management systems.
  • Understanding of the needs of older people, particularly those from CALD backgrounds. A second language will be highly regarded.
