Matilda Nepean Hospital is a 90-bed sub-acute flagship hospital nestled in a world-class medical hub at the base of the Blue Mountains in Western Sydney, adjacent to Nepean Public Hospital. Matilda Nepean Hospital is seeking an experienced full-time social worker to join our team, working closely with the entire Multidisciplinary team including a full-time discharge planner.

Criteria:
- AASW Registration
- Minimum of 1-2 years of post-graduate experience
- Experience working in Mental Health, Rehabilitation and/or Palliative Care
- Experience managing clients with complex needs
- Experience managing NCAT, NDIS, MAC and other services
- Demonstrated understanding of evidence-informed practice and critical thinking
- Sound verbal and written communication skills
- Ability to work independently and as part of a cohesive team
