Your opportunity: The Clinical Nurse Consultant will lead a multidisciplinary team for Aboriginal, Torres Strait Islander and vulnerable populations with or at risk of developing Type 2 Diabetes Mellitus in persons aged 15 years and older. • Better Cardiac Care (BCC): Provides Aboriginal and Torres Strait Islander people culturally safe, multidisciplinary support to help them manage heart disease and transition smoothly from the Mackay Base Hospital back into the community and prevent readmissions.
  • Potentially Preventable Hospitalisations Team (PPHT): Provides Aboriginal and Torres Strait Islander people with culturally safe, multidisciplinary proactive interventions and early disease management in the Sarina community.
  • Together Strong Connected Care (TSCC): A multidisciplinary secondary/tertiary prevention service providing culturally safe, evidence-based healthy lifestyle education for Aboriginal, Torres Strait Islander and Australian South Sea Islander people who are at risk or living with Type 2 Diabetes Mellitus.
