NSW Age Care IW - Registered Nurse - Community Nursing
Make a Difference in Community Care Lumia Care is seeking an experienced and compassionate Registered Nurse to join our growing Community Nursing team, supporting clients across the Inner West, Sydney CBD, and St. George regions.
This role is ideal for a nurse who enjoys working independently, delivering high-quality clinical care in clients' homes, and building meaningful relationships within the community.
About the Role As a Community Registered Nurse, you will provide care to clients funded through DVA, SAH, Private, and Brokerage programs. You will be responsible for delivering a range of clinical services while ensuring exceptional client outcomes and person-centred care.
Preferred Start Date: 30 November 2026
Availability Required: Monday to Friday, with Tuesday being essential
Key Responsibilities
- Conduct comprehensive client assessments
- Deliver wound care and chronic disease management
- Provide clinical nursing interventions in the community
- Develop and review care plans
- Monitor client progress and escalated concerns where required
- Maintain accurate clinical documentation
- Liaise with clients, families, allied health professionals, and wider care teams
About You
To be successful in this role, you will have:
- Current AHPRA registration as a Registered Nurse
- Community nursing experience highly regarded
- Demonstrated experience in wound care and clinical assessments
- Strong clinical decision-making and problem-solving skills
- Excellent communication and interpersonal skills
- Current Australian Driver's Licence and reliable vehicle
- National Police Check (or willingness to obtain)
- Current First Aid & CPR certification
