About the Role
We're looking for an experienced Registered Nurse to provide clinical leadership and hands-on nursing care to NDIS participants with complex health needs. Working on the ground alongside our support worker teams, you'll get to know the people you support and play a genuine part in improving their quality of life.
This role suits a nurse who wants more autonomy in how they work, real relationships with participants, and the satisfaction of making a tangible difference every day.
What You'll Do
- Conduct comprehensive health assessments and develop individualised care plans
- Train staff in and oversee medication management
- Provide clinical supervision and support to support workers and the wider team
- Monitor participants' health status and report changes to the medical team
- Support the onboarding of new participants and review care plans as needs change
- Identify, investigate and manage clinical incidents in line with policy and do NDIS reporting where required
- Liaise with participants' medical teams, families and other stakeholders
- Travel to SIL, SDA and participant homes to deliver hands‐on care
- Maintain accurate, timely clinical documentation and NDIS reporting
Requirements
- Nursing qualification with current AHPRA registration as a Registered Nurse
- Sound understanding of NDIS requirements and managing complex health needs
- Excellent communication skills and the ability to build strong relationships with participants, families and team members
- Current First Aid and CPR certification
- Valid driver's licence and access to a reliable vehicle
- Yellow Card, Blue Card and Police Clearance
