As a Support Coordinator, you will:
- Hold your own caseload of NDIS participants across Swan Hill and the surrounding districts.
- Help participants understand their plans and connect them with the right providers and community supports.
- Build participants' capacity to make informed choices about how their funding is used.
- Monitor plan budgets and act early when an underspend or overspend is developing.
- Maintain accurate case notes, service agreements and records in line with the NDIS Practice Standards.
- Keep billable hours at or above 80% of your scheduled hours.
- Coordinate with families, carers and other professionals involved in each participant's support.
To be successful in this role, you will bring:
- A relevant qualification in community services, social work, disability or allied health, as required for registration.
- Experience as a support coordinator in the NDIS.
- Clear written and verbal communication with organised caseload habits.
- A current NDIS Worker Screening Check and Working with Children Check, or willingness to obtain both before starting.
- A current driver licence and a reliable vehicle for participant visits.
- Full working rights in Australia.
You are someone who takes ownership of your caseload and does not wait to be told when things need attention. You write notes that reflect what happened, follow up on outstanding matters, and communicate clearly with participants, families and providers without overcomplicating it. You are comfortable working in a small regional team where everyone has real responsibility, and you value the kind of support that helps you do your job well rather than just check a box. You show up, care about the people you work with, and hold yourself to a high standard.
What you will get:
- A permanent part-time role, four days a week.
- Paid travel time between participant visits and a kilometre allowance for your own vehicle under the SCHADS Award.
- Regular supervision so you're supported, not left to figure things out alone.
- Direct access to leadership in a small regional team.
- Meaningful work with a caseload that is yours to own and grow.
Your Support Coordination is a regional Victorian provider of NDIS support coordination, working with participants across northern Victoria. The team is small, meaning every person has real responsibility and a genuine impact on the people they support. You won't be lost in a big organisation; you'll have direct access to leadership, real supervision and a team that takes the work seriously.
