JJ Care Services is seeking a Care Partner – SIL & Complex Care to provide RN-led clinical governance for residents of our dementia and complex medical needs home, plus a caseload of high-intensity community clients living in their own homes.
You will translate assessments into funded service plans, develop safe, well-supervised care on the ground, and collaborate with a Workforce Planner to focus on clinical delivery and staff training.
