You will assess referrals, plan care, manage budgets, and collaborate with agencies to meet customer goals.
Hybrid work arrangement with at least three days in the nearest IRT office and two days from home, across multiple locations in Qld, NSW, ACT and beyond. 1–2 years aged care experience preferred.
