- Location: Serving Ballarat and surrounding areas
- Travel: Must be willing to travel for client care

- Week 1: Mon 0700 -1430, Tuesday 0700 - 1430 (14hrs)
- Week 2: Mon 0700 -1430, Tuesday 0700 - 1430, Sunday 0700- 1100 (18hrs)

Role Responsibilities & Qualifications:
- Provide health promotion and education to clients and their carers.
- Promote and deliver our award-winning Wellness for Independence® programs, both in-person and virtually.
- Monitor and improve health outcomes through Telehealth.
- Direct Care Services: Provide care across various funding programs, including wound care, diabetes management, medication management, stoma care, and catheter care.
- Centered Care: Assess, develop, and evaluate nursing care plans collaboratively, focusing on client goals and independence with support from Allied Health clinicians.
What you need to be considered for the role:
- Understanding of community nursing and primary health care principles, including working within a consumer-directed care framework.
- At least 2 years of experience in community, acute, hospital or aged care.
- Experience working independently or as part of a team.
- Strong organizational skills and attention to detail.
- Proficiency with electronic devices and client management systems (smartphones, tablets, laptops, email, etc.).
- Current Registration with AHPRA, Police check, working with children check and CPR.
- Experience providing advanced clinical care, including chronic disease, medication, palliative care, catheter management, stoma care, wound care, and diabetes management.
