Hunter New England Local Health District is seeking a passionate Nurse Practitioner for Residential Aged Care—In Reach. Based at Manning Base Hospital, you will deliver acute care to residents in aged care homes, using face-to-face and virtual consultations to ensure right care at the right time.
You will work with a multidisciplinary team, lead clinical decision making, and contribute to a growing RAC-I program across Newcastle, Lake Macquarie, Port Stephens and beyond, enhancing outcomes.
