This is a hands-on role that requires regular visits to our residential houses to support frontline teams, monitor service delivery, address operational needs, and ensure young people receive safe, consistent, and person-centred care.

Working closely with the Regional Manager and operational teams, you will play a key role in care coordination, young person intake, staff support, stakeholder engagement, and service quality. • Support the development, implementation, and review of individualised care plans to meet young people's needs and goals.
  • Coordinate young person intake, assessments, onboarding, and transitions into services.
  • Build and maintain positive relationships with young people, families, guardians, support coordinators, and external stakeholders.
  • Provide guidance and operational support to Assistant Coordinators, House Seniors, and frontline support staff.
  • Monitor service delivery, identify risks, and escalate concerns to ensure safety and well-being of young people.
  • Maintain accurate care plans, Authority to Care documentation, and service records.
  • Support rostering and workforce coordination to maintain appropriate staffing levels and continuity of care.
  • Promote compliance with Child Safety Standards of Care, relevant legislation, and organisational policies and procedures.
  • Contribute to audits, quality assurance, and continuous improvement initiatives.

What We're Looking For

  • Demonstrated experience in Youth Work, Child Protection, Disability services, Residential care, or a similar Community Services environment.
  • Experience coordinating services, supporting operational delivery, or managing care-related processes.
  • A sound understanding of the NDIS framework, service delivery, and compliance requirements.
  • Strong stakeholder engagement and relationship management skills, with the ability to communicate effectively with young people, families, staff, and external providers.
  • Excellent organisational and problem-solving skills, with the ability to manage competing priorities in a dynamic environment.
  • The ability to identify risks, respond appropriately to concerns, and elevate matters when required.
  • A collaborative and professional approach, with the commitment to safe, person-centred support and positive outcomes for young people.
  • The ability to work independently while contributing positively to a multidisciplinary team.
  • A current Australian driver's licence and reliable transport.
