About the Role
As the Clinical Care Coordinator you will be responsible for the provision of optimum levels of care to patients, through effective leadership, management of staff and other resources, and the development of cooperative professional relationships.
- Lead and mentor a team of Registered Nurses, Enrolled Nurses, and Care Staff.
- Oversee care planning, assessments, and clinical governance.
- Ensure compliance with Aged Care Quality Standards and legislative requirements.
- Work collaboratively with residents, families, GPs, allied health, and community partners.
- Support workforce planning, rostering, and performance management.
- Promote a culture of safety, respect, and continuous improvement.
- Contribute to strategic planning and service development.
About You
You will be a motivated and compassionate leader with:
- Minimum 3 years' experience managing clinical care in an aged care setting
- Strong people management and leadership skills
- Excellent communication skills and an engaging, supportive leadership style
- AHPRA Registration
- High level of organization and adaptability, with a commitment to innovation and excellence
- Experience mentoring and developing nursing staff
- Willingness to relocate to Regional Victoria
