Practice Information

This multidisciplinary service provides community-based support to people experiencing severe and complex mental health, psychosocial, behavioural and care coordination needs across rural and regional communities.

Based within a general practice, the service offers intensive, recovery-oriented care for people who may experience barriers to traditional mental health services because of geographic isolation, complex presentations, psychosocial vulnerability, fragmented services or co-occurring challenges.

The service model incorporates outreach, multidisciplinary collaboration, care coordination and integrated care with local health, social and community services. The position reports to the general practice administrative team.

Position Description

As a psychologist within the multidisciplinary team, you will provide psychological assessment, therapeutic intervention and coordinated support to people experiencing severe and complex mental health presentations. You will manage a complex and varied caseload, deliver discipline-specific care and work collaboratively within a general practice environment.

  • Providing comprehensive screening, psychological assessment, care planning, therapeutic intervention and care coordination
  • Identifying clinical deterioration, escalating risk, safeguarding concerns and urgent support requirements
  • Supporting assertive engagement strategies to improve continuity of care and access to services
  • Supporting people who may be at elevated risk, vulnerable to disengagement or experiencing significant social complexity
  • Escalating or de-escalating care appropriately across stepped-care and acute services
  • Participating in multidisciplinary case conferencing, risk management and coordinated response planning
  • Developing collaborative relationships with hospitals, primary care providers, community and crisis services, and external agencies
  • Contributing to integrated care planning for people with co-occurring mental health, physical health, behavioural, housing and social support needs
  • Delivering flexible outreach support, including community-based appointments and home visits
  • Attending and facilitating appointments at regional clinic hubs
  • Providing care in non-traditional environments to support engagement and continuity of care
  • Maintaining high-quality progress notes, documentation and reporting in accordance with professional and program requirements

  • Gathering relevant information for psychiatrist review and multidisciplinary planning
  • Participating in quality improvement, evaluation, service development and professional supervision
  • Practising in accordance with trauma-informed, recovery-oriented, culturally safe and person-centred frameworks

The role involves managing a high-complexity clinical caseload in a rural outreach environment. The successful psychologist will exercise substantial professional judgement, independent decision-making and advanced care coordination skills while contributing to multidisciplinary care planning and system navigation.
