We are seeking a passionate and experienced Support Coordinator to join our growing Disability Services team on a part-time basis, working four days a week.
About the Role
You’ll work alongside NDIS participants to build their capacity, strengthen their independence, and support them to navigate and maximise their NDIS plans; providing coordination across informal, community and funded supports, ensuring participants achieve their individual goals while maintaining choice and control. This is a rewarding opportunity for someone who is client-centred, organised, and outcomes-focused.
What You’ll Bring
- Experience in NDIS Support Coordination, disability services, or a related human services role
- Strong understanding of the NDIS framework, supports and service pathways
- Excellent communication, relationship-building and problem-solving skills
- Ability to manage a caseload, prioritise tasks and work autonomously
- High level of professionalism, integrity and empathy
- Relevant qualification in Community Services, Disability, Social Work or similar (desirable)
- Current driver's licence, NDIS Worker Screening Check and Working with Children Check (or willingness to obtain)
