Are you someone who thrives in a fast-paced environment, loves solving problems, and genuinely enjoys making a difference in people’s lives? H1 Healthcare is growing, and we’re looking for an experienced Care Coordinator to join our Sydney team. In this role, you’ll coordinate disability and in-home support for clients across NSW, with a strong focus on participants funded through Icare NSW. You’ll juggle competing priorities, build strong relationships, and make sure every client receives the care they deserve in their own home and community. No two days are the same. One moment you’ll be coordinating complex supports, the next you’ll be finding creative solutions, liaising with families and Icare case managers, or heading out to visit a client. If you’re organised, adaptable and love a challenge, this could be the perfect role for you.

What You’ll Be Doing

  • Coordinating personalised, person-centred support plans that help clients live independently at home and achieve their goals
  • Managing complex care arrangements for participants under Icare schemes, including Lifetime Care and Workers Care, in line with Icare guidelines and standards
  • Visiting clients in their homes across Sydney and NSW for annual reviews, assessments, and whenever needs change
  • Working closely with clients, families, support workers, Icare case managers and allied health professionals to deliver seamless, high-quality care
  • Rostering and matching support workers, and responding quickly to changing client needs
  • Preparing service requests, progress reports and documentation to Icare standards and timeframes
  • Advocating for clients so they can access the right funding, services and community resources
  • Monitoring service quality and client safety, and responding to incidents and feedback
  • Problem-solving, thinking on your feet and making every day count

What You’ll Bring

  • Demonstrated experience working with Icare programs (essential)
  • Experience in disability, attendant care or in-home support coordination
  • A solid understanding of Icare service request processes and reporting requirements
  • Exceptional organisational, coordination and rostering skills
  • Outstanding communication and relationship-building abilities
  • A proactive, solutions-focused mindset with the confidence to make decisions
  • The ability to manage competing priorities without losing attention to detail
  • Willingness to travel within NSW to visit clients annually and as required
  • A current driver’s licence and access to a reliable vehicle
  • A current NDIS Worker Screening Check and Working With Children Check (or willingness to obtain)

Highly regarded:

  • Experience in the disability, or worksafe sector
  • Experience supporting clients with complex or high-needs care
  • Qualifications in disability, community services, health or allied health

Why You’ll Love Working With Us

  • A varied, rewarding role where no two days are the same
  • The opportunity to work with complex and specialised care programs
  • A supportive leadership team that has your back
  • Ongoing professional development and career progression opportunities
  • Travel allowance / vehicle reimbursement
  • The chance to be part of a respected national healthcare provider that’s continuing to grow
  • A workplace where your ideas, initiative and passion are valued
