This permanent full-time leadership opportunity offers the chance to shape the delivery of aged care assessment services across a large and diverse regional catchment while influencing service development during a period of significant sector reform. The role is open to managers with relevant healthcare backgrounds such as nursing, physiotherapy, occupational therapy and social work in particular.

From July 2026, GV Health’s Aged Care Assessment service operates within the Victorian Department of Health Lead Provider Service Model, partnering with Latrobe Community Health Service and Gateway Health across the Vic 4 Service Area. This role provides a unique opportunity to lead local service delivery while contributing to regional planning, governance and performance across the partnership.

As ACA Manager, you will provide strategic, operational and clinical leadership to a multidisciplinary team delivering high-quality aged care assessments across Greater Shepparton, Strathbogie, Moira, Mitchell and Murrindindi areas. You will oversee service performance, workforce development, quality and risk management, budget management and stakeholder engagement, and ensure the service remains responsive, effective and consumer-related. Working closely with the Divisional Operations Director and other leaders, you will influence service innovation, lead continuous improvement initiatives and support the implementation of contemporary aged care reforms that improve outcomes for older people across our region.

About you:

You are an experienced clinician and leader who is passionate about improving services for older people and supporting teams to deliver exceptional care.

Successful candidate will have:

  • Extensive clinical experience within aged care, complex care, community health or a related field
  • Demonstrated leadership experience, including managing and developing high-performing teams or a readiness to step into a management role
  • Strong knowledge of clinical governance, quality improvement and risk management principles
  • Experience in service development, strategic planning and change management
  • Excellent communication, stakeholder engagement and partnership-building skills
  • The ability to work collaboratively across health, aged care and community sectors
  • Commitment to team and self-development
  • A commitment to leading through GV Health’s CREATE values
  • Registration with AHPRA as a Registered Nurse, Occupational Therapist or Physiotherapist, or eligibility for membership with the Australian Association of Social Workers
