The ForHealth Group is seeking an experienced Part-Time Chronic Disease Management (CDM) Nurse for our Forest Hill practice. You will support GPs and patients to identify and manage chronic conditions, with a focus on preventative care and healthier outcomes.
As an AHPRA-registered Enrolled Nurse or Registered Nurse, you will prepare care plans, conduct health assessments, empower patients and coordinate care with a multidisciplinary team.
