At ermha365, we work with clients with complex mental health and disability support needs in Victoria. We’re a lifeline for people who often feel ostracised or excluded from the simple things that most of us take for granted. We believe in the potential of everyone. We never give up, and we never give in.

The Support Facilitator’s goal is to empower rather than to do; to promote independence, development and confidence in the participant. Ideally, we are looking for a candidate with Lived Experience or non-Lived Experience who has the ability to:

  • Provide psychosocial support to young people and adults with severe mental illness on a short to medium term basis (up to 12 months)
  • Deliver a range of evidence-based mental health interventions to individuals
  • Manage a caseload of 10-15 participants providing support with mental health & wellbeing goals
  • Conduct bio psycho-social assessments of individuals, including risk and protective factors associated with mental health outcomes
  • Collaborate with participants, clinicians, and other stakeholders (e.g., family) to access and engage with various support services
  • Develop, monitor, and track recovery goals, implementing steps to promote independence
  • Conduct screenings and assessments, and participate in case planning and exit reviews alongside care teams and clinical staff
  • Advocate for individuals and build sustainable support networks

This role works on a hybrid system, including working from home, in our Geelong office and assertive outreach support to participants across the Greater Geelong region and Bellarine Peninsula. Program hours are typically 8am to 4pm with flexibility to 8pm. This is a maximum term part-time (0.8 FTE) contract position ending 30/06/2027.

It is a requirement of this position to meet all compliance requirements concerning NDIS Work Screening, Victorian Working With Children's Check, working rights and possess a current & valid Victorian driver’s licence.

Additional attributes we are looking for include:

  • Previous experience in assertive outreach, youth work, case management, mental health, justice or AOD
  • The ability to effectively and quickly build rapport with people and think outside-the-box to achieve positive outcomes
  • Flexibility, patience, confidence, initiative, street-smarts and emotional resilience
  • The ability to contribute positively within a team and work autonomously to manage your caseload
  • Demonstrated capacity to work flexibly, manage competing demands, and effectively set boundaries
  • Qualifications in psychology, mental health, social work or other relevant fields (required)

We take appropriate actions to ensure that our staff and the people we support are as safe as possible. You will be expected to adhere to all OHS standards, requirements and processes.

When you join as Support Facilitator, you can expect:

  • Support as needed utilising our 24/7 Employee Assistance Program
  • Industry-leading induction and on the job training provided with access to free continuous online learning modules at your own pace
  • Flexibility in working hours to suit work-life balance
  • Financial benefits including salary packaging to reduce your taxable income, novated leasing and discounts with a variety of Australian retailers
  • A supportive team environment that is engaging and inspiring with the same vision and values
  • Access to Fitness Passport, making it easier to stay active and healthy as we care about your health and wellbeing
