ermha365 is seeking a Link Worker in Geelong and Portarlington to help people build social connections and engage with community activities. The role uses lived experience and training to support consumers, families and carers, promoting social inclusion and social prescribing in daily practice.
Based in Geelong three days a week and Portarlington two days, the position is a 12-month fixed term, with a strong focus on reducing isolation and strengthening community participation.
