Direct Care Resources is seeking an experienced Case Manager (Manager for Care Planning and Practice) to lead care planning and frontline practice across Child Safety & Disability Support services. The role is based in Murarrie with regular client-home visits, providing guidance to teams to ensure safe, individualized care.
You will report to the Senior Manager – Care Planning and Practice and work with support workers, operational teams and health professionals to translate care plans into practice.
