Build the SIL program. Own the growth. Lead the outcome.

Crescent Care Foundation is a registered NDIS provider based in Liverpool, delivering Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), high-intensity supports, specialist support coordination and allied health across Western Sydney. We have the registration, the homes and the service depth. What we need now is one exceptional person to own the full SIL growth journey — from the first conversation with a referrer through to a participant moving safely into a home that’s right for them. This is not an intake desk role, and it is not a cold-calling sales job. It’s a senior, accountable position for someone who has done this before, can show the results, and wants to build something that lasts.

The role As SIL Growth & Intake Manager, you’ll be the single accountable owner of Crescent Care’s SIL pipeline and conversion. You will:

  • Build referral channels from scratch — support coordinators, specialist support coordinators, hospital and health discharge teams, allied health, behaviour support practitioners, guardians, advocates and community organisations
  • Run a disciplined pipeline in our CRM — referral, funding verification, assessment, property visit, compatibility, service agreement, transition and commencement
  • Qualify and match well — only progressing participants whose funding, needs, preferences, risks and housing requirements we can support safely and sustainably
  • Lead the transition process — property visits, co-tenant consultation, compatibility assessment, transition planning and stakeholder communication
  • Partner with operations and clinical leadership on workforce mobilisation, rosters, participant‑specific training and handover
  • Report to the executive on channel performance, qualified opportunities, conversion, forecast commencements and lost‑referral insights

What you bring:

  • 3+ years in NDIS SIL business development, intake, or home-and‑living roles — with a track record you can evidence
  • Demonstrated SIL conversions you personally generated: referrals sourced, property visits, signed agreements and actual move‑ins
  • Referrer relationships you’ve built from zero, not just inherited
  • Working knowledge of SIL funding, roster feasibility, compatibility, tenancy separation and service agreements
  • Experience with some of: hospital discharge, psychosocial disability, complex health, behaviours of concern, guardianship or justice interfaces
  • Strong understanding of the NDIS Code of Conduct, conflict‑of‑interest obligations and the new SIL Practice Standards
  • Excellent judgement — you know when not to proceed with a placement, and you can explain why
  • Current NDIS Worker Screening Check (or willingness to obtain), WWCC, driver’s licence and own vehicle
  • Community language skills (e.g. Arabic) highly regarded

How we work:

We grow the right way. Every referral is consent‑based and transparent, and participant choice comes first. Admission decisions are made through a multidisciplinary panel, so you’ll never carry clinical risk alone. We don’t pay for referrals, we don’t pressure families, and we won’t ask you to bring a former employer’s database. We’re hiring your capability, not a contact list.

What we offer:

  • $110,000 – $130,000 + superannuation
  • Capped performance incentive linked to quality outcomes: suitable commencements, 90-day participant retention, documentation and participant experience
  • Genuine seniority and authority, with direct access to the Director and executive team
  • A clear path to leadership as the SIL portfolio expands
  • Structured onboarding with 30/60/90-day goals agreed together
  • Hybrid flexibility, with a culturally diverse, values‑led team in Western Sydney

Crescent Care Foundation is an equal opportunity employer. We strongly encourage applications from Aboriginal and Torres Strait Islander people, people with disability, and people from culturally and linguistically diverse backgrounds. All appointments are subject to NDIS Worker Screening, a Working with Children Check and reference checks.
