Community Services provides quality home support services to the general community and specialises in services to the CALDB (Culturally and Linguistically Diverse background) community in the Brisbane and South Coast areas.The agency has been responding to individual and daily needs of the aged in the community since 1978 and aims to always provide a service of excellence that is responsive to client's individual circumstances.
We are seeking a Care Services Coordinatorwho ismotivated, reliable, compassionate and organised for our Commonwealth Home SupportProgram (CHSP) in our Brisbane office.
Thisfull time positionisresponsible for coordinating support services to eligible clients by using a team of in home care staff.
Overall the Care Services Coordinators' responsibility is to provide quality care services that are compliant with the agency's policy and procedures and the Strengthened Age Care Quality Standards.
The successful applicantwill beliaising with clients, family members and staff to ensure services are provided in accordance with the client'sneeds, goals and preferences.
Requirements:
TertiaryQualifications/Diploma in CommunityServices/ Cert IV in Ageing Support
Demonstrated knowledge of the Aged Care Act andCHSP guidelines
Strong communication skills and the ability to model a positive culture through a professional service
Attention to detail, ability to follow processes and manage a high volume of clients in a fast paced environment
Have an understanding and experience in delivering services according to the Strengthened Aged Care Quality Standards
Demonstrated understanding of the changing and diverse needs of clients
Proven relationship management with internal and external stakeholders, as well as good interpersonal and negotiation skills
Ability to communicate in another language
Current National PoliceCertificateissued by the Australian Federal Police Canberra
