Capital Health Network (CHN) is seeking a dedicated Relationship Coordinator to lead coordination and implementation of Mental Health MDT services in Canberra. This part-time position supports planning, design and commissioning of the MDT model of care, and manages contracts to meet ACT community needs.
The role works with CHN-funded providers across primary care and community services, including ACCHOs, and requires strong relationship management to ensure seamless integration.
