Better@Home is seeking interested clinician Enrolled or Registered Nurse to join their team on this fixed term contract. This position will report to Better@Home Manager. The aim of the position is to provide support to patients across their care continuum, working with patients and their care teams, identifying strategies or services to reduce the need to come to hospital.
The suitable candidate must:
