This full-time, Monday to Friday role focuses on person-centred care and supporting older Australians to stay safe, healthy and independent at home.
Reporting to the Coordinator - Aged Care Services, you will:
- assess needs
- coordinate care
- manage wound care and medications
- collaborate with families and multidisciplinary teams
