Burnie Brae is seeking a compassionate Registered Nurse | Clinical Care Partner to join our Support at Home team. Based from our Chermside office, this hybrid role balances home visits, office work and community care to support older people across North Brisbane and Moreton Bay.
You'll manage a portfolio of up to 50 clients, perform clinical assessments, care planning and medication management, and work with a multidisciplinary team to deliver person-centred care in clients' homes.
