BlueCare is seeking a Care Partner to provide person-centred, high-quality community care in Gympie and surrounding areas. You will oversee care planning, coordinate services with clinical teams and support clients across their aged care funding, with on-the-road visits a core part of the role.
With a Certificate IV in Health or Community Services and a current Australian Driver's Licence, you'll manage care plans, budgets and liaise with teams to respond to changing needs.
